Best tools to quickly build a prototype website app with database capabilities?

Looking to prototype a website application that can handle signup/login, survey questions and using a database infrastructure that is easily migrated to MySql or something more robust down the road. Anyone work with a tool that can enable the fast build of a web application?

I'll second the Wordpress.

Annette, All modern tools are more than adequate and to call one of them best is a matter of personal taste. The best tool is the tool you already know, even if there's something newer. PHP, RoR, Python/Django or even Scala/Lift all viable, modern and will get you good results if you know how to use them. I'd concentrate on finding the tech team you can work with and let them use the tools they already know.
I've never personally used it, but I know some people who have used https://bubble.is/ to rapidly build web apps visually (drag and drop).

It seems you are getting lots of suggestions. Atomic.io is very new and good tool for prototyping UX quickly and cheaply - it enables you to build working models of UC to get the feel for what apps will work like without going to all the trouble of building the full database linkages and fully functional HTML. This may not be what you are looking for but I thought it might be useful to have the suggestion anyway since you are talking about prototyping.
